New Patients

Thank you for choosing Selby Acupuncture. We are excited to begin working with you! Here is some information that you may find useful as a new patient to our practice. If you still need to get your appointment set up, you can book online or call our office at 651-224-6678.

Selby Acupuncture Intake Form

Your initial intake forms for acupuncture treatment are available on the email confirmation that you receive after your appointment is booked. Please fill those out before your visit so they upload to your electronic file, along with any insurance information that may be appropriate.

If you are coming for a Shiatsu treatment, please arrive 10-15 minutes before your appointment to fill out the Shiatsu Intake form.

Welcome Letter

After booking your appointment, you can also review our welcome letter after filling out your intake forms. In this letter we try to answer questions you may have before you begin your treatment at our practice. Please look it over and let us know if you have any questions.

HIPAA and Consent to Treatment

You will receive a copy of our offices HIPAA (Health Insurance Portability and Accountability Act) policy in your confirmation email. This explains how we keep your personal information secure. We also will have a Consent to Treatment form which explains the different treatment methods that we commonly use, along with any side affects you may experience. You are able to sign both of these forms electronically. If after reviewing these forms you have questions, just give us a call or let us know at your visit and we can help.
